At first glance, the Google Pixel Watch 5 looks a lot like its predecessor. The circular display, the familiar crown, and the comfortable fit all remain. But this year’s refresh is less about physical changes and more about what the watch can do through software. The new model starts at $399, reflecting a $50 price increase over last year’s launch price. It’s a noticeable jump, but Google is hoping that a deeper push into AI and proactive health will make the Pixel Watch 5 feel like a meaningful upgrade.

Under the hood, there’s a slightly faster Qualcomm processor, a modest battery bump, and a new satin pyrite case finish. There are also new strap colors and a Steph Curry Special Edition. But the biggest updates this year are all software-based. That approach echoes Google’s recent Fitbit Air launch, which also leaned heavily into AI-driven health insights. With the Pixel Watch 5, Google is trying to show that the future of wearables isn’t just about hardware specs — it’s about what the watch can do with the data it collects.

Gemini comes offline and gets more proactive

One of the most notable upgrades is that Gemini, Google’s AI assistant, can now work offline on the watch for simple tasks. This means you can start timers, launch apps, or perform basic commands without needing a cellular or Wi-Fi connection. During an early demo, the offline mode was noticeably slower than when Gemini was connected to the internet, but the feature still worked as promised. It’s a convenient addition for people who want quick assistance during workouts or in areas with poor connectivity.

Gemini is also becoming more proactive. In one demonstration, a text message asking for availability brought up a suggestion to check Google Calendar directly from the wrist. The assistant is designed to surface relevant actions before you even think to ask for them. There’s now an icon on the watch face that shows real-time information like timers, Uber ETAs, and workout progress. This makes the watch more glanceable and reduces the need to dig through menus.

The popular pinch and wrist-flick gestures, which first arrived through a software update in December, are now more deeply integrated into Wear OS 7. These gestures let you answer calls, dismiss notifications, or scroll through widgets without touching the screen. They feel more natural on the Pixel Watch 5, and the updated interface makes better use of them.

AI-generated watch faces

Another fun addition is the ability to generate watch faces using AI. In a demo, the prompt “fat cats and an elegant font” produced a watch face with plump, cat-shaped clock numbers rather than actual cat characters. It’s a gimmicky feature, but it highlights Google’s effort to bring the Nano Banana model to the wrist. If you enjoy customizing your watch with unique, whimsical designs, this lets you create something personal without needing design skills. It’s not a core reason to buy the watch, but it adds personality to the wearable experience.

Breathing Emergency Detection

The most significant health feature is Breathing Emergency Detection, which builds on last year’s Loss of Pulse feature. When the watch detects a severe drop in blood oxygen saturation, known as SpO2, it can automatically call emergency services and notify your designated emergency contacts. This could be life-saving in situations where someone suddenly stops breathing.

To reduce false positives, the watch combines heart rate, motion, and altitude sensors with SpO2 readings. During a demo, Google noted that the feature won’t activate unless SpO2 drops to around 83 percent — a dangerously low level — for an extended period. It also disables itself once the watch determines you’ve been asleep for 30 minutes, which helps prevent accidental alerts caused by sleep apnea or side sleeping. The feature is not intended for people with chronic respiratory conditions; it’s designed to flag acute instances where a person may be struggling to breathe. It will be available on both the Pixel Watch 4 and Pixel Watch 5 in Europe at launch, with FDA clearance still pending in the United States.

Sleep breathing quality trends

Google is also adding nightly sleep breathing quality tracking. This feature will monitor how well you breathe while sleeping and provide monthly updates. The goal is to help you see how daily habits, like exercise, alcohol consumption, or stress levels, affect your respiratory health over time. Unlike emergency detection, this is a long-term wellness metric rather than an urgent alert. It’s a useful addition for people who want to understand their sleep patterns and make adjustments to improve overall health.

Blood pressure and insulin resistance trends

Two of the most talked-about health features are blood pressure trends and insulin resistance trends. It’s important to note that neither of these is diagnostic. They’re wellness tools designed to act like a “check engine light” for your body. They require at least a week’s worth of data and will flag monthly trends rather than providing real-time readings. These features won’t be available at launch; Google says they’ll roll out starting in September. They’ll also come to the Pixel Watch 3 and later models, as well as the Fitbit Air.

For blood pressure trends, the watch uses its optical heart rate sensor and motion sensors to measure blood flow. The data is fed into an AI model that estimates the shape and speed of pulse waves traveling under the skin. This helps the watch determine whether your blood pressure is trending higher or lower over time. It won’t give you specific systolic or diastolic numbers, but it can alert you to potential changes that might warrant a check-up with a doctor.

Insulin resistance trends are even newer. Insulin resistance is a condition where your body doesn’t metabolize food effectively, leading the pancreas to produce more insulin to manage blood glucose. Left untreated, it can cause fatigue, stubborn weight gain, and eventually lead to Type 2 diabetes. The Pixel Watch 5 will passively estimate whether your insulin resistance is trending up or down month to month using signals like heart rhythm, sleep quality, and daily movements. Google says it validated the algorithm against blood draws and published the results in a peer-reviewed study in Nature.

This is not noninvasive blood glucose monitoring. There are no direct glucose readings, and it’s not a screening tool for diabetes. Instead, it’s meant to flag sustained metabolic stress that might otherwise go unnoticed. Insulin resistance can remain asymptomatic for years and often doesn’t show up in routine metabolic tests. A traditional diagnosis usually requires fasting insulin and glucose blood tests. If this feature works well in real-world conditions, it could be a major step forward for preventative health.

Fitness upgrades: better GPS and AI strength coaching

Google is also making bold claims about GPS accuracy. Fitbit and Pixel Watch users have long complained about unreliable GPS maps from outdoor activities, especially in dense urban areas. Last year, Google introduced dual-frequency GPS. This year, the Pixel Watch 5 will use satellite data, weather station information, and 3D building models from Google Maps to correct GPS data using AI in post-processing. Google says this will allow the watch to outperform dedicated fitness watches like the Garmin Fenix 8 and Apple Watch Ultra 3. That’s a strong statement, and it remains to be seen how well it works in practice.

Another fitness upgrade is AI-generated strength training plans that can be accessed entirely from the wrist. This feature was already available through Google Health Premium, but previously you needed your phone to log reps or move to the next exercise. Now, everything can be done on the watch. This is a smart move, especially as more people incorporate strength training into their routines. The ability to follow a structured workout without pulling out your phone makes the experience much smoother.

Software-first approach and pricing

The Pixel Watch 5 is a clear sign that Google is doubling down on software and AI rather than hardware upgrades. That might disappoint people who were hoping for a new design or more sensors. But it also means that older Pixel Watch models will benefit from many of the new health features. Most of the health software updates will be backward-compatible, so owners of the Pixel Watch 3 or 4 have little reason to upgrade right away.

The $50 price hike, however, makes the upgrade decision more complicated. At $399, the Pixel Watch 5 faces stiff competition from other smartwatches. Google is betting that AI-driven health insights, offline Gemini, and proactive suggestions will justify the higher price. It’s a calculated risk. The health features are genuinely impressive in scope, but they’re also relying on algorithms that have yet to be tested by millions of users.

Ultimately, the Pixel Watch 5 is a software story. It’s about making the watch smarter, more helpful, and more focused on long-term health. The new case finishes and the Steph Curry Edition are nice touches, but the real value lies in features like Breathing Emergency Detection, insulin resistance trends, and offline AI support. Whether those features live up to their promise will depend on real-world testing, but Google has laid out a clear vision for where it wants to take its wearables.
